How to Request a Report (Insurance Companies)
Requesting an Autopsy Report
You may request a copy of the final Report of Examination by sending an original or photocopied standard medical information release form that has a notarized signature of the next-of-kin and is dated within the previous 90 days. A copy of a Report of Examination, which includes toxicology results, requires a fee of $35 paid in advance (make checks payable to the Utah Medical Examiner's Office). Requests along with the check can be mailed to our office.
For another option, you or your client may also print a Records Request Form and have it signed by the next-of-kin and notarized. We must receive the original or a copy of this document along with proper payment.
Toxicology results and other testing may require a prolonged period of time before results become available and the final report is issued.
If processing of a policy simply requires a determination of whether or not foul play was a factor in the death, you may contact our office (801-816-3850) during regular business hours. If available, you can speak to the physician assigned the case and get a verbal response to this determination. If the physician is not available, leave a message with return contact information, including extension numbers, and we will return your call as early as possible.
