Can you tell me information about the death for an insurance policy?
We can give some information about a death to insurance agencies, but other information is only available in the report of examination.
Call our office at 801-816-3850 if you need to know if foul play was a factor in the death. The medical examiner who is working on the case can give you a verbal response to this question.
What is a report of examination?
A report of examination has detailed information from the deceased person’s postmortem exam and death investigation.
We only give copies of this report to the deceased person’s immediate family members, police officers, attorneys, and health care providers. We can give it to insurance companies who have permission from the deceased’s next-of-kin.
How do I get a copy of the report of examination?
Mail a copy of a completed records request form by the next-of-kin and a check or money order for $35 payable to the Utah Office of the Medical Examiner.

We’ll send a copy of the report to you within 3 business days of receiving your request. If the report isn’t finished when we get your request, we’ll send you a copy of the report when it’s ready.
